Informed Medical Decisions, Inc. (dba “InformedDNA”) (“We,” “Us,” or “Our”) provides genomics-enabled clinical and advisory services to payors, life sciences organizations, and health systems. We are committed to protecting the privacy of individuals who engage with us. This Privacy Policy describes how we collect, use, and safeguard information across our website, digital platforms, telephone communications, and in-person interactions (the “Services”). When we handle protected health information (“PHI”) in connection with our clinical services, we do so in compliance with the Health Insurance Portability and Accountability Act (“HIPAA”). For other interactions, we process information in accordance with applicable privacy laws and our internal policies.

WE MAY MODIFY THIS PRIVACY POLICY AT ANY TIME, SO CHECK BACK REGULARLY FOR UPDATES. ALL CHANGES WILL BE EFFECTIVE IMMEDIATELY UPON POSTING TO THE WEBSITE. WE WILL ATTEMPT TO NOTIFY YOU OF ANY MATERIAL CHANGES VIA THE WEBSITE OR THE PLATFORM.

Legal, regulatory, and compliance-related sharing

  • Regulators, law enforcement, or other governmental authorities when required to comply with applicable laws, legal processes, or regulatory obligations.
  • Parties involved in protecting rights and safety, such as when we believe sharing is necessary to detect, investigate, or prevent fraud, security incidents, or potential harm.

SITE ANALYTICS, COOKIES, AND OTHER TRACKING TECHNOLOGY

We use analytics services and targeted advertising cookies in addition to location features that are connected to your use of the Services. We use cookies, web beacons, pixel tags, and other technologies (collectively, “Cookies”) on our Services to automatically collect information that helps us improve the Services and advertise to you.     We use third-party software tools on our Sites to learn more about how visitors navigate the Website and view content.     • Google Analytics and Marketing: We have enabled and implemented Google Analytics, Google Tag, Google Marketing, Google Analytics and Google Dynamic Remarketing, a Google Analytics Advertising feature. To prevent Google from using data collected by Google Analytics, follow the instructions to download and install the Google Analytics Opt-out Browser Add-on for each web browser you use. Using the Google Analytics opt-out browser add-on will not prevent us from using other analytics tools and will not prevent data from being sent to the Site itself or to Google.     • FullStory. We use FullStory, a behavioral data analytics platform, to help us better understand how users interact with our website and digital services. FullStory captures information such as clicks, page views, scrolling, and navigation patterns, which allows us to analyze usage trends and improve the overall user experience. The data collected may include technical information about your device and browser, but it does not include sensitive personal information such as passwords or financial details.   2. Targeted Advertising
    We use service providers and other third parties to serve you customized email, SMS, and internet-based advertisements. Those ad networks use cookies and other tracking technologies to serve ads based on your prior visits to our Website. These ad networks may place and store cookies on the devices you use to access our Services and collect information, including your browsing behavior, referring websites, and your response to advertisements.
    • Meta Custom Audiences: We use the remarketing services of Meta, Inc., 1601 S. California Ave, Palo Alto, CA 94304, USA (“Meta”) on our Site. We use Meta Custom Audiences to deliver targeted advertisements to website visitors across those Meta owned social media platforms based on email addresses that we have collected. YOUR PRIVACY CHOICES

    • Update your Personal Information. You may update your Personal Information by contacting us using the information in the “Contact Us” section below or by contacting us at compliance@informeddna.com.
    • Delete Your Account. If you no longer wish to use the Services or access your account or account information, you may request us to delete your account by contacting us at compliance@informeddna.com. This will result in the full, permanent deletion of your account and associated information. We will not be able to recover your account after it has been deleted.
    • Update Your Browser Settings. If you wish to limit or opt out of third-party advertising, you can do one or more of the following:
      • The Options/Settings section of most internet browsers will tell you how to manage cookies and other technologies that may be uploaded to your device, including how to disable such technologies. You can disable our cookies, or all cookies, through your browser settings, but please note that disabling cookies may impact some of the features of our Website and prevent them from operating as intended. SOCIAL MEDIA

If you are logged into a social media account while using our online Services, the social media platform may associate your activity on our Services with your social media account. We do not control how these platforms collect, use, or share your information.

DE-IDENTIFIED / AGGREGATED DATA

We may aggregate or de-identify Personal Information we collect through our Services and when operating our business. For example, we may aggregate Personal Information to calculate the percentage of users in a particular zip code. Once information or data has been de-identified and/or aggregated, such information is no longer Personal Information.

CHILDREN’S PRIVACY

InformedDNA’s website is not directed at children under 18. If you are under 18, do not use or provide any Personal Information to InformedDNA. If we learn that we have collected or received Personal Information from a child under 18 without parental consent, or if you become aware that we may have such information, alert us immediately at compliance@informeddna.com.

LINKS AND THIRD-PARTY SITES

Our online Services may contain links to websites or content operated by third parties. These websites are not controlled by InformedDNA, and this Privacy Policy does not apply to their privacy practices. If you choose to visit a third-party website, any information you provide or that is collected by that website will be governed by the third party’s own privacy policies and terms. CALIFORNIA’S SHINE THE LIGHT ACT

If you are a California resident, under California’s Shine the Light Law (California Civil Code Section 1798.83) you have the right to request the following information regarding our disclosure of your Personal Information to third parties for direct marketing purposes: (1) the categories of Personal Information we have disclosed to third parties for their direct marketing purposes during the preceding calendar year; (2) the names and addresses of the third parties that received the Personal Information; and (3) if the nature of the third party’s business cannot be determined from their name, examples of the products or services they marketed.
